+Provenance: reconstructed 2026-10-26 from the two feedback reports that reference this file
+(`RPT-20261023-165659-9a6835ff`, `RPT-20261023-165656-29d32e82`). Both said "rationale written to
+`_docs/decisions-2026-10-23.md`", but the file was absent — the decisions themselves were only
+recorded inside those reports. Text below is verbatim from the reports.

+## Section 10 — predictor store
+
+**Decision:** Go per-user. Server file is canonical; the browser VFS is a TTL read-through cache,
+written through on every save and read only when the server file is unreachable. The divergence
+banner fires only when the cache is newer than the server, or the server is unreachable.
+**Remove the second independent write path** — two writers is what produced the garbage pair in the
+accuracy store.
+
+## Section 6 — dispatch
+
+**Decision:** spawn-on-demand; do **not** build the A2A mailbox — `_agents` has no mailbox dir and
+`delegate_task` already auto-spawns. Change `Dispatch 0/3` to count delegations rather than queue
+picks, relabel it as on-demand with a count of idle workers, and increment it inside `delegate_task`.

+## Consequences for the feedback queue
+
+- The predictor garbage-pair report (`RPT-20261023-153550-61b448d3`) is a *symptom of the second
+  write path*: removing it is the fix, not cleaning the pair by hand.
+- Section 6 has no mailbox work to schedule; any plan task that creates `_agents/*/mailbox` is void.
